I have this issue for more than a year and i have no idea what it could be.

The problem:

Since i bought this new SSD from Kingston the same SSD will randonly disapear from Windows Explorer, sometimes causing a BSOD with "DRIVER_POWER_STATE_FAILURE".
Right after that happens, some times the windows will restart and the SSD wont show up (even in bios) and everything will work normaly (without the SSD).
Sometimes after the BSOD the windows will try to restart and be stuck at windows logo loading screen (right after bios screen) which the only solution is to remove the SSD and the system will start without a problem.

What i think/did:
  1. At first i thought it could be a virus or something, i did a clean Windows install and same thing would keep happening randomly.
  2. I thought it could be my PSU, but i'm able to run any game at 100% gpu and 100% CPU and everything works perfectly so i discarted the possibility of PSU problem.
  3. I thought it could be the SSD, so i RMA it to kingston which sent me a brand new one and you know... same thing keeps happening... so i don't think the SSD is the problem (unless i'm really unlucky and got a second SSD with the same problem)
  4. installed a few intell hardware management software that i saw from a few random topics with similar problems in the internet and no luck.
Well, i'm running out of options....could it be the motherboard? I really hope not since i have a really old system (MOBO: Asus P8h61m, GPU: GTX 970 STRIX, CPU:i72600k, 8gb ram) and theres no LGA 1155 mobos anymore around here.

Any thoughts? Thank you.

EDIT: typo

SSD shouldn't just vanish. Windows has no control over BIOS so it not showing there is curious.
Do you have the latest BIOS for motherboard?

Driver power state error is what makes me wonder if its software. Not sure what drivers Kingston ssd would have though.
I would run this on the drive - https://www.kingston.com/en/support/technical/ssdmanager

Can you download and run Driverview - http://www.nirsoft.net/utils/driverview.html

All it does is looks at drivers installed; it won't install any (this is intentional as 3rd party driver updaters often get it wrong)

When you run it, go into view tab and set it to hide all Microsoft drivers, will make list shorter.

Now its up to you, you can look through the...

Now its up to you, you can look through the drivers and try to find old drivers, or you can take a screenshot from (and including)Driver name to (and including)Creation date.

upload it to an image sharing website and show link here

All I would do is look at driver versions (or dates if you lucky to have any) to see what might have newer versions.

Can you follow option one on the following link - here - and then do this step below: Small memory dumps - Have Windows Create a Small Memory Dump (Minidump) on BSOD - that creates a file in c windows/minidump after the next BSOD

  1. Open Windows File Explore
  2. Navigate to C:\Windows\Minidump
  3. Copy the mini-dump files out onto your Desktop
  4. Do not use Winzip, use the built in facility in Windows
  5. Select those files on your Desktop, right click them and choose 'Send to' - Compressed (zipped) folder
  6. Upload the zip file to the Cloud (OneDrive, DropBox . . . etc.)
  7. Then post a link here to the zip file, so we can take a look for you . . .

Hi @Colif , thank you for your reply!

Doing the steps you told me to:
1) Kingston software: I installed it a few times already and it always says everything is working as intended.
2)Driverview: View: https://imgur.com/wD33ALs

3)BIOS: from cmd looks like i'm using 0501 (which seems outdated https://www.asus.com/supportonly/P8H61-M/HelpDesk_BIOS/) i'll try to update it as soon as i can.
4)Dump files: https://drive.google.com/file/d/1EMILn2mRscqVniTZic8Mc8vw6Gh9Acdz/view?usp=sharing

2 I seen driver power state failure errors caused by Nvidia drivers recently
download Win10/Win11 Auto Installation Program (NDIS) from under windows header here - https://www.realtek.com/en/componen...0-1000m-gigabit-ethernet-pci-express-software

no intel drivers listed.

3 that might help, it might update it to point mb knows what an ssd is

are you booting off the ssd? Support for ssd is spotty with older boards and many don't even support booting from them, as the size of ssd back in 2012 wasn't the same as it is now, they were mostly used for cache drives. 32gb drives that are now too small for windows (and yet people try).

4 I ask a friend to convert dump files.

Thank you again @Colif for your time.

I've installed the NDIS from realtek and also intel/chipset driver which you pointed out, hopefully it helps.

About booting from a SSD i currently have a 120gb Sandisk SSD that i've been using for over 3 years for my windows (never had any problems before) and this new 'problematic' SSD (the kingston one) is only used as storage/games so i don't boot from it.

The BIOS update i'm kinda scared about it since i never did it before, so i kinda hope its the last resource. I know i shouldnt, but still.

Finally, about the dump files, i've opened some of them in WinDbg to try and see if i could find some info myself but no luck :p Better leave it to the pros.

I'll wait a bit and see if the NDIS and chipset helps with the problem, if not i'll try some other nvidia drivers and finally, this weekend if its still happening, i'll update my bios.

Let me know if you find anything in the dump files. Thank you sir!

PS: I'll also update here if anything new happens or if the problem is solved.

Update: After doing the things above i got a new BSOD today, VIDEO_TDR_FAILURE. Never got any of those before, not sure if related to the drivers i installed or just coincidence. I can add the dump file here if needed aswell. It happened while watching a youtube video.

Hi, I ran the dump files through the debugger and got the following information: https://jsfiddle.net/ktg17o6L/show This link is for anyone wanting to help. You do not have to view it. It is safe to "run the fiddle" as the page asks.
File information:121121-12390-01.dmp (Dec 11 2021 - 21:48:52)

NOTE: There are multiple revisions of your motherboard. Be sure that I have the correct one. There is a BIOS update available for your system. Wait for additional information before deciding to update or not. Important: Verify that I have linked to the correct motherboard. Updating your BIOS can be risky. Never try it when you might lose power (lightning storms, recent power outages, etc).

This information can be used by others to help you. Someone else will post with more information. Please wait for additional answers. Good luck.

Thank you for your message aswell @gardenman hopefully someone have some insight on the problem.

About the mobo, its indeed the P8H61-M LX3 model, i accidentaly posted the link to the wrong one before. My bad.

By the way, got the TDR BSOD again today, with the same symptom, BSOD, PC stuck at windows logo screen after mobo screen. After i remove the ssd the PC boots normaly.

Its really weird, like it decided to show a new error now for some reason instead of the old one :(

Maybe it could be the PSU? It has 10 years now but i still dont have problems when playing at 100% gpu and 100% cpu, makes no sense to me. (its this one: https://www.newegg.com/seventeam-st-750zaf-750w/p/N82E16817176004)

I have noticed that adding new hardware can cause older hardware to give up.
I added a new GPU to my last PC and my speakers that had worked for last 12 years decided to die. But it wasn't that obvious, it took a few weeks to figure out. I added new GPU and suddenly desktop wallpaper wouldn't load. I naturally thought it was the thing I had changed, not something else. It was only after I moved PC to another room, reinstalled windows, found it worked fine and then plugged speakers back in to have it start again; that I realised the actual cause... and cried a little (they were good speakers)

What keyboard do you have? one of the dumps could be a keyboard to blame.

So new ssd that disappears isn't C. Meaning all errors on the old ssd. except ones caused by new ssd being in PC

I feel new one is bad, i don't think it being in PC would cause the errors alone, it may have pushed something else.

All 3 crashed Chrome, I would traditionally blame LAN drivers
May 24 2019rt640x64.sysRealtek NICDRV 8169 PCIe GBE Family Controller driver https://www.realtek.com/en/

there will be newer

Hi @Colif thanks again for the reply.

My keyboard is an old Coolermaster Quickfire TK , it is connect through a KVM with my Logitech G903 so i can use them both at my PC and PS5.

I'll try download new LAN drivers.

I currently disconected the new SSD just to double check the new BSOD errors and since them i havent have any BSOD, just like previously, without new SSD everything seems to work fine.

But i got some news, not sure if related but today when i turned on my PC i got a warning bios screen telling me that my Windows SSD that i currently have for around 5 years is going bad (SMART is going bad, backup files to avoid possible loss or something like that).

Given the message i installed a few HDD/SSD monitoring tools to check him out and he is indeed not looking so good (pictures below).

I'm not sure if my old SSD (Sandisk) that contains my SO having problems would really do something like the symptoms im having with the new SSD (Kingston) plugged in.

I'll be trying a few things that crossed my mind:
  1. Try a new sata and power cable with the problematic SSD (Kingston) (maybe one of the cables is bad?)
  2. Install windows in the new SSD (Kingston) and only use it to see if problems still happens.
Anyway, thats all i can think for now, if you guys have any other suggestions let me know, and thanks again for your time.

Another update.

Like 1 hour ago (even without the new SSD) my PC just went into another TDR BSOD and this time when it restarted it was reparing the files, the repair process froze for like 30 minutes so i just said <Mod Edit> it and pressed the reset button.

After a few restarts i was back at my desktop, decided to do something more 'wild' , turned off my PC, removed the old SSD that had SMART problems, added the new SSD that was out of the computer and installed Windows in it.

Will update if the problems stop or not.

All ssd have spare memory they can use to replace sectors if they go bad, but the pool of free sectors isn't infinite and if ssd doesn't have any spares, it can't replace bad sectors. That could be what top one is warning about

So yours is reporting 4 bad sectors, running chkdsk can make windows mark them as bad and not use them

But a 120gb ssd that is 5 years old is about due for retirement. At least its warning you instead of just dying. We normally suggest a bigger ssd for windows now, even if it doesn't use all the space. Advantage to bigger drives is more spare free sectors, and you can use drive forr more than just windows and not feel squashed

Most ssd have more memory on them that they give you access to. a 120gb ssd will likely have 124gb on it, the remaining 4gb of memory used for things like error correction. The bigger the ssd, the bigger the amount of memory left for this function. 1tb ssd have 24gb free, for instance

Some ssd let you put memory aside for more error correction, I put 91gb aside on my 1tb nvme. It probably doesn't need it but I don't either. space isn't something i use or lack. If it means drive lasts longer, good.

What make/model PSU? I know you already thought of it, but just curious as if its same age as PC, it could (maybe) be reason why new ssd stopped working... it might not be. Could be just a bad ssd.

Hi @Colif , thanks for all the information.

My PSU is this one that I posted previously:
https://www.newegg.com/seventeam-st-750zaf-750w/p/N82E16817176004

The bad boy has over 10 years. Maybe she could be going bad?

Since i dont have any problems when running games at full load, i didnt think the PSU could be bad.

I didnt have much time using the PC with only the new kingston ssd with Windows in it, like 3 hours tops, but no problems so far
