Well, I'm not sure if my GPU is dying or whether or not it's the drivers. I've uninstalled the drivers and re installed them, but the issue will still occasionally happen. Basically if I'm playing a game (or watching a video, or sometimes just on the desktop) my screen will flicker, and then blue/black horizontal lines will fill the screen. After the computer goes back to normal, I'm given a message at the bottom-right of the screen saying "AMD display driver has stopped working, windows has resolved the problem". Obviously, it didn't resolve anything. I tried to screenshot the lines, but the screenshot reveals that the screen just looks normal (obviously not a good sign).
I know people have reported dying cards and that they show artifacts, but when I'm playing a game, I don't get any of these. Generally the screen just flickers and then the striped lines appear. I'm really confused as to if this is because my card is potentially dying, or if I need to somehow wipe everything related to AMD off my PC and install everything again. My card is still under warranty, so getting a new one isn't a huge deal, but if I can save it I'd like to.
I've attached my build below as well as a screenshot of the Catalyst Control center to give you and idea of the clock speeds on the GPU itself. The temperature never goes over 75 degrees on any game, so it's not overheating.

Here's a small YouTube video showing the flickering. The screen didn't totally stripe out this time, just flickered and went back to "normal". Ignore the Persona 4 emulator. It crashed when I tried it once, so I thought it might do it again.
