I have:
AMD R8 m445dx
AMD quad core A10-9600P
8 GB ram
Windows 10

Fallout 4 lags even in low settings, mostly during combat. Why is this? Is my laptop that bad? Fps doesnt seem to change when using higher setting like medium or higher.
It's always 30-45 fps everywhere and sometimes 60 FPS depending where I am. But I don't really care, I want to have good fps during combat. Everytime I am on combat, my fps is like 15-20 it's frustrating. I always die easily to these gulpers, anglers and deathclaws even though I have high defense and kiloton radium rifle with better mods and perks. I can't aim properly without good fps and thats why I die so fast. And the combat lags mostly happens outside (duh I mentioned the swamp monsters). I'm fine combating inside because the lags are lessened.

Is there a fix? A mod? I have some performance mods installed and they seem to help but not in combat. I'd appreciate it if you guys find me a solution to this.

And for extra info, I did edit the .ini files because the game feels like crap due to the sensitivity, its as if you were playing on a console with a mouse.

The underlying issue is that you're on an AMD APU, not a CPU. The designation "R8 M445DX" refers to the combination of the R5 chip on the A10 and the separate dedicated R7 M440 GPU; it's not nearly as powerful as a true dedicated GPU, even in mobile form. Unfortunately, since you're on a laptop, it's what you're stuck with; as you found, lowering settings is more or less the only option.

You should also turn down shadows and grass, if you haven't already; rendering those two aspects is one of the heaviest rendering tasks in games these days simply because of the sheer amount of grass and the fact that shadows are dynamically generated based on calculating the direction a light source is striking an object from.
