Solution
Maybe you were stressing out the PSU and it finally had enough of being over stressed and normalized it's self at which point stopped the card from functioning properly.

Either way, for sure upgrade that PSU.

Normally if you are under the correct wattage it simply wouldn't even turn on the display to the monitor.
Try to reinstall your graphics drivers. I've seen windows 10 actually remove graphics drivers during updates or attempt to install drivers from windows updates instead of using manufacture ones. Check in device manager to make sure you dont have any missing drivers. They will display an ! icon if they do.

While you are doing this. I would first use the DDU tool and completely remove all drivers then perform a fresh install of only your GTX 560 drivers.

okay, did as u said, uninstalled everything and then reinstall.
but the same problem is till here.
tried running crysis 2 on everything ultra and i got 5 fps. i noticed that my cpu was at 32% usage as well as my ram being on 40% so it dosnt seem like those are the culprits . tried virus scan, also nothing . is there a program to see how much the gpu is under pressure perhaps?

Sounds like you are running off the CPU's internal graphics processor. Is the fan spinning on your GPU? Maybe try removing it and re-installing it to make sure it is secure in the Pcie socket.

Edit: Unless you physically removed the cable from the monitor to the GPU and hooked it up to the motherboard's video port, what I said above doesn't make sense. Can you verify the monitor is connected to the GPU?

went into bios, disabled integrated graphics and still nothing 😀 this is turning into a major headache. thanks for u guys help tho ! :) really grateful

gonna try the stress test now to see if anything happens

im shifting through a varaity of games to be sure im not only having issues with one game 😉

Are the other sensors in GPU-Z registering values? Fan speed, GPU temp, memory used, etc. That would verify that GPU-Z is actually working.

My GPU utilization % is varying between 22 - 23% with 2 monitors on, watching TV on one, and Chrome running on the other.
