GPU Has stopped performing (Dramatically)

Hi all.

So I have an average computer in my lounge, which I mainly use as a Media PC.
The huge problem I'm having is that the GPU Has all of the sudden dropped in performance. I have been running a stable clock on it for the last 6months, using MSI Afterburner, and it has been fine. it has never gone above 80-85 degrees C, (under load) but all the sudden i started up the PC the other day and noticed that in game it was horrible!
So I proceeded to run a stress test to see whats going on and I noticed the following:
Gpu Temp idle's at 30-35 deg C, when put under full load in stress test it only went up about 5 deg C?!?!
then I also noticed that all though both GPU usage, and FB usage were both between 95-99% (which is good under heavy load), the VID usage and the BUS usage were flat lined at 0%!! Also Core, Shader, and memory clocks were all running quite low as well. All being used but not to their fullest under a stress test load. This has resulted in a drop from usually 45-60 FPS to a shocking 5-15 FPS which is impossible to use! its only a 560Ti (I am not very well off haha) but has always worked swell for what i use it for and how i use it. I have made no hardware changes, so I dont no what the heck is going on! :( PLEASE HELP???

I am quite Hardware savvy, but when it comes to software I am hopeless.
so please dumb down your answers so that I can understand what it is I need to do?

I had an issue with my 570 similar to this. The core clock would go ridiculously low and it wouldn't go back up until restarting the PC. One day the restart didn't help anymore, and the next day it was dead. I'm not saying that your card is dying, but it could be a possibility.

Hi thanks for your reply, It most definitely could be a possibility. I would hate to think so though. it just seems strange that it came out of nowhere, didn't even start playing up first.
