[SOLVED] GTA V Lags after playing for a while

PC Specs:
CPU:Core I7 960 3.20 GHZ
Ram: 6 GB
GPU:GT 220 3789MB Total Memory (Looks way wrong)
Windows 7 Ultimate 64 Bit

The game works fine for like 15 minutes or so, even driving at high speeds does not cause lag but after a while the game becomes unplayable. It might be a memory leak but I don't know for sure
I used CPUID HwMonitor to check temperatures and took some screenshots.

Solution
Get yourself 3x 4GB 1600MHz ram (Gskill), more ram will help a lot reducing hdd activity substantially. A better graphics card would go a long way as well, Gtx 1660 Super would suit if you have the power supply for it.

An SSD for the OS + GTA5 will also be highly beneficial.

GTA5 does go through a lot of system resources so having more ram, better gpu and a faster system drive accelerating pagefile performance will see day/night difference. Go for more ram first and then see.
