GTX 960 Frames Very low..

I'm really frustrated with this card.. I have a GTX 960 2GB ASUS, I5 2400, 8GB RAM, I have high performance on both the GTX control and the power options for windows.. I bought a brand new 500 EVGA PSU. When I run Ryse Son of Rome, For honour on any settings from low to high.. I'll start at 60fps and will stay like that for 5 minutes or so then drop can go from 60-20 in seconds and then eventually it just stays in low frames.. When I check HW monitor my GPU memory and GPU core stays 90% and above close to 100%.. doesn't that usually mean bottleneck but shouldnt my cpu be fine and not bottlenecking..
 
Considering you have a pretty old CPU and also some other factors, there are two possible (or maybe three) possible problems:
The CPU is the bottleneck in general. (Then you'll want to replace it, if you're looking for more frames)
The CPU, after five minutes, heats up too hot and starts to thermal throttle (especially considering the age). (This means you'll want to check the temps) (MOST LIKELY PROBLEM)
There could be a driver problem - more unlikely.
