It depends on the title. Medium is probably right where you can expect to be, in most cases .

Not really sure what your question is.

I run a 980 ti at 2560 x 1440 on a 144hz monitor at Ultra for almost all my games. I sacrifice fps because I place a high value on how a game looks, but my games don't drop below 60 fps.

If you want to know what settings are going to get you 144 frames, then I'd be inclined to say that medium is your best bet.

Like @Neurodrive said, it really depends on what your frame rates are. I'm running my 980ti on a 34" Predator (1440p @ 100 Mhz in 21:9) and generally run them all at Ultra or High (depending on the game). I never have a FPS issue, especially since the GSync keeps the FPS smooth.
