Question Help me pls HDD Critically low health.

Hi! I recently ran into a problem for the last few weeks my pc kept crashing and lagging so crazy. Then I tried troubleshooting it and it kept crashing until I cant enter to starting screen, it kept on trying to system repair but it only says WE RAN INTO A PROBLEM YOUR COMPUTER IS NOT STARTING PROPERLY. So I tried formatting my computer see if it works. I successfully entered into the starting screen and started checking if the problem was gone. I run task manager and the disk active time was 100% and it barely has any programs running. So I found out the root of the problem it was my HARD DISK so started asking how to fix it. I tried to:

Run CHKDSK
Defragging
Changing the HDD cables
Cleaning the disk

But none of them works so I asked my father what to do he said try monitoring your drive and to install Hard Disk Sentinel . I installed it and I saw the HDD health is extremely low at 9%

How do I fix it? I'm still at Junior High School I don't really know things this Advanced pls help me.

How do I fix it?

You prepare for a full replacement.

Get anything you want to keep off that disk and hopefully it stays together long enough to complete that.
