Is my GPU dying?

Alrighty, this is the last place i can come to for an answer. My specs are:
EVGA 1070
i7 6700k @4.00
16GB RAM

I have this glitch in a few games that i have been playing, Witcher 3 and Rainbow 6: Siege.
Its making Witcher 3 almost unplayable, but its only in very small locations in siege. The GPU is only 2 months old, so i'm unsure if I have some kind of mismatch in my settings, or its just dying.

I have tried the following to address the issue:
Fresh install of drivers
Fresh install of both games.
Re seating my GPU.
Changing the graphics setting to low and medium in game.
Disabling all background processes other than necessary.

I have talked to nVidia support, Witcher support and some reddits, and no one has had this issue other than me that i can tell. At this point i think my GPU is just dying.



 
Solution
You need to get it seen to. I wouldn't even leave it on with voltages like that. You could check all power cables, but you have to get that fixed as it is definitely the cause of your problems.
you can eliminate drivers and windows as a culprit if you boot to a linux USB/DVD distribution and test the card there.
puppy linux is tiny and can run entirely from RAM, mint Linux is very friendly to windows users and should be familiar.

if during testing you get artifacting it would be hardware.
 


PUS is EVGA SuperNOVA 850 B2, 80+ BRONZE 850W, and the GPU wont fin into the other PCIE slot on my mobo, to much stuff, not enough room. I dont know how to check system voltages.

Your voltages are seriously off-whack. Your 12v is 0.048. It should be 12. Your vCore is half what it should be. They are all WAY too low. You have a serious issue with either your board or PSU. Do you definitely have all power cables attached correctly? Did you build the system yourself?
 


Nope, had a local PC place build it for me. Should i take it in and explain the issue or is there something i can do to fix this?
