Question Monitor Screen Freezes/black out while playing games but CPU keeps running

My issue started appearing 2/3 days ago,whenever i play any games for like 10-15 mins a sudden blackscreen/freeze happen with "GRrrrrrrrrrrrrr" sound from headphones and CPU fans keep running and nothing work after that unless i do a hard restart,Sometimes i have also noticed that my GPU spiked at 100% and this also freezes my monitor.If you have any solution please help me.

My PC specs :
Motherboard : MSI B450 Gaming Plus Max
CPU : Ryzen 5 3600X
GPU : GeForce RTX 2060 Ventus
RAM : Corsaur Vengeance 16GB DDR4 3200Mhz
HDD : Seagate Sata 1TB Internal (after few month added more 4TB)
SDD : Western Digital 500GB M.2 NVME SN550
Power Supply : Corsair : VS650
Cooler : Cooler Master Hyper 212
Cabinet : Cooler Master K501L
Main Monitor : 144hz MSI Optix G241
2nd Monitor : 60hz BENQ GW2283
Operation Sys : Windows 10

I have been playing Fortnite and i keep my task manager open on 2nd monitor to see the temp when it happens and usually whenever a black out or freeze happen temp is around 77-79 degrees and i recently noticed that gpu % sometimes goes till 100%
