My NVIDIA driver keeps crashing. Screen goes black. Sometimes I get the message "Input signal out of range".

How do I fix this? My computer would randomly crash. Sometimes during gaming or sometimes just randomly while browsing. There are two types of crashing though:


    1. Screen would just suddenly go black. No warning. I can still hear my music. So, it's just the video card. Sometimes the driver would recover after crashing and goes back to normal. I have the most recent driver.



    2. Everything would stop in the background, music would stop playing and I will get the error message, "INPUT SIGNAL OUT OF RANGE, CHANGE RESOLUTION TO 1920 x 1080 60 Hz."
    The settings are already on 60Hz and same resolution.

CPU: i7 3770
Mobo: Maximus V Gene
RAM: 12 GB (DDR3), DRAM Frequency: 668.5 MHz
GPU: GTX 960 (GDDR5), Core: 135 MHz, Memory: 405 MHz

What should I do? I have tried re-installing the drivers.
