PC Graphic Card 4K Help?

Hi, I just had to do a factory reset to my PC because I was having some problems with it, ever since I done that reset I can't seem to get my 4k monitor to work correctly. I'm using the DisplayPort which came with the monitor. (http://www.samsung.com/us/computer/monitors/LU28D590DS/ZA) it will only allow me to go up to 3200 x 1800 resolution as shown in the picture: (http://gyazo.com/f834c88b5df9381133eb865c179a8533) I'm not sure if it's something to do with CCC (Catalyst Control Centre) but I can't seem to get it to 4k resolution. Please help.

GPUs: Sapphire Radeon R9 290
CPU: Intel i7 4790k.

Yes I re-installed the drivers and I've gotten the lastest 14.12 Omega. Also my monitor did come with a driver, I've just downloaded it and restarted my PC but nothing seems to have changed. Here's a picture of my CCC version. :http://gyazo.com/f67527b061215af136690f8d0da212b5

I don't know if this is any help but on CCC it says my resolution is at 2560x1440 at 75Hz but Control Panel says I'm running a 3200x1800 at 60Hz and my monitor says I'm running at 2560x1400 60Hz / 88.9kHz
