Wrong sata port? broken sata cable? Anti virus running in background? Wrong sata drivers?

None of those... I'm going to replace it. I'm pretty sure it's simply junk.

Everything is perfect and triple checked. Used different sata ports/cables. Nothing running in background.

If you know your BIOS settings, cabling and driver configuration is proper, then it's probably a bum drive. Any of those issues can cause sluggish performance, but your numbers are abnormally low.

The only thing I'd be leery of is if you migrated your HDD to your SSD instead of doing a fresh install.

It was a fresh installation, and everything is perfect. I'm getting a new one. I will update this when I do.
