Screen Tearing with V-sync On

I'm getting screen tear in games even with V-sync turned on. I'm getting a solid 60 fps.
I can "fix" it by setting an unlimited frame rate in game, but that keeps my CPU hammered at 100% and causes the fans to go at full speed. In addition, the tearing isn't fully gone, just less noticeable.
Any help would be appreciated.


Settings

NCP:
V-Sync: Fast
Triple Buffering: On

In-Game:
Vsync: On
Frame Rate Limit: 60fps


Specs:
i5 4690k
gtx 980 ti
8GB ram

Set graphics for maximum. Then turn down grass detail, viewing distance as those are cpu heavy settings. Put physX on gpu not cpu. Set in game fps for 90. Tearing is the result of the gpu starting a new frame before the refresh of the monitor has finished the last one, so you'll need to punish the gpu and slow it down as best as possible. The addition of a 4k DSR if possible will help.
