Question Screens turn off - Audio cuts in and out - GPU fans max to 100%

Around the start of March last year I started having a problem where my screens would shut off and my GPU fans would max out at random times whilst playing any game, I tried a bunch of stuff to fix it (listed below) but nothing seemed to work, after about a week of the issue it seemed to have fixed itself however the next month it did it again. This would basically keep happening until July where after it had happened I then experienced no problems with it for the rest of the year until today. was playing high GPU usage games all day with no problem until 5 hours ago, I changed literally nothing, no updates no touching anything in the computer and it just randomly decided to do it again. The last driver updates I did to it were about a week ago and just as a precaution I made a restore point to go back however that hasn't changed anything.

Currently the GPU runs at about 70-80℃ and Hotspot can get up to 100℃ but it has been doing so fine for the past 4 months.

CPU temps are around 50-60℃ and the 4 ram sticks are at about 40-50℃ and the 2 m.2's sit in the mid 40-50℃'s.

Below is the list, that's about all I can remember trying, basically have done it all.

What I have tried:
  1. Reinstalling drivers (with and without DDU)
  2. Reinstalling Windows
  3. Cleaned and Dusted out the computer
  4. Reapplied thermal paste to the CPU and GPU
  5. Done a MemTest64
  6. Test each memory stick 1 by 1 (issue persisted on all 4)
  7. Updated RAM Firmware and Software
  8. Updated BIOS
  9. Updated Chipset Drivers
  10. Checked Event Viewer for Errors (no errors)
  11. Running both Case and GPU fans at 100%
  12. Used Armory Crate to update parts of the Motherboard and GPU
  13. Switched out Power Supplies
  14. Switched out GPU
  15. Reseated the GPU higher and screwed in tighter to try and reduce sag
  16. Turned XMP on and off to try and reduce power draw
  17. Changed Windows power plans
  18. Changed Monitor refresh rates
  19. Ran a Prime95 test on the CPU and RAM
  20. Used HWInfo to collect a minidump to see what the temps were on crashing (same as usual)

Motherboard: Asus Rog Strix x570-F Gaming Motherboard
CPU: AMD Ryzen 7 3700x
CPU Cooler: NZXT Kraken X62
RAM: 4x Corsair Vengeance Pro 8gb 3200mhz C16
GPU: Asus Rog Strix 2060 Super OC
Storage: 1x Samsung 970 EVO 250GB - 1x Samsung 970 EVO 500GB
PSU: Be Quiet! 500w (Forget what exact model)
Case: NZXT H500
Monitors: Asus Rog Strix XG258 240hz - Asus VG258 144hz
 
This actually sounds like a failing PSU more than anything else, at this point. How old is yours?

Please get the exact model off the label on the side of the PSU and post that info here.
Exact model is a Be Quiet! Straight Power 11 500w, would say it is about 4-5 years old.

I did try a Thermaltake smart pro 500w and I was still experiencing issues.
