Does anyone know how to fix this ?

I have tried the chkdsk /f & sfc /scannow in safe mode but nothing

Frequent or widespread file corruption on a hard drive is often a sign that the drive is failing. Get your data off it while you can and save it elsewhere. Then test the drive for confirmation on it's state of health with DOS-based testing software which loads from a bootable CD. The links are here: http://www.tacktech.com/display.cfm?ttid=287

Make a CD from the ISO file using IMGBurn: http://www.filehippo.com/download_imgburn/

Then boot your faulty PC from that CD.
