[SOLVED] Signal disconnecting and crashes... graphics card issue?

Somewhat recently, I've started getting this issue that seems to occur in a few different places:

Basically what happens is I'll turn on my computer, my monitors turn on, and then my monitor has a black screen for up to a minute before it shows my lock screen. (it used to be literally a few seconds)
The other time it does this is when I change my screen resolution in a game. I'll click to change the resolution and the screen will go black, turn on and off multiple times (the monitor), display "no signal", and then the process repeats for a minute until it has adjusted (this also used to take seconds).

This is another weird issue I'm having that I think could be related to the previous:
Sometimes after my computer has been on for at least an hour, all of my monitors will turn white with vertical lines (typically blue or gray). After about 10-15 seconds the audio I had playing stops and it doesn't shut down by itself like a normal bluescreen crash, so I have to hard shut-off/reset. I don't think it's an overheating issue because when I run a resource-heavy game it won't ever do it, only when I'm doing something simple like watching Netflix or looking at sites like this.

I originally thought this was a problem with my old hdd, but I tried replacing it with a new ssd and I'm still getting the problem. I've also tried updating drivers, using different monitors/different cords, and I don't have another card/pc to test with.
