There needs to be an permanent tohit penalty/ceiling to such
things. No matter how good you are a setup like this will always be
harder to hit something with.

As usual, it depends on the gameplay.

Or are you arguing from realism?

You could, for example, make the ammunition so precious, that you'll
use this kind of setup only if it's going to be a quick head-on attack
on a large group of strong enemies, exactly like in Alien.

But wait, why would you want to discourage using it, once you made so much
effort to actually implement it in your game? ^-)
That would make duct tape some kind of scroll of enchant weapon (well, it
would certainly look interesting after a few such 'enchantments').

You could also attach non-weapon elements, like a lunette, laser-aiming,
grenade launcher, motion scanner, silencer, microwave oven, cd-player,
net launcher, additional cooling system for your plasma, etc.

And, with the duct tape, you can do it even when the part is from
a different weapon 😉

NEVER!

It was more a concept to be allowed for... normally a poor weapon
like this can simply be compensated for by a huge natural tohit
bonus to the PC's dex. I think it would be fun and add depth that
such a cobbled together solution would misfire or have some misadventure
a small % of the time (not necessarily missing the target - but for
example the tape might come apart - or catch fire etc).

In such cases the weapon needs a separate stat that affords a
failure rate not effected by any such PC bonuses - or in any
case the ability to add such a stat should be thought of in
advance (now we are thinking about H-World and Carceri and
allowing the engine this ability).

It could be as simple as 5% of the time this weapon misses its
target regardless. This 5% is added at least cumulatively for
each new weapon you try to duct tape to the others!
