run command fixmbr it sounds like the boot record and table have to be rebuilt/repaired. then reboot and try again, let me know what happens

If the computer boots and tells you a file is corrupted or missing use the repair mode in a Bootable USB or DVD windows disc. Otherwise if you know how to generally fix computers and such do what popatim stated

Ok so you know the card works. Have you tried using the display on another computer? Or a different display input (Dvi, hdmi, vga)? If you try another input and it works, try the normal plug you use on another computer. If it fails see if the cord is bad by swapping it, if you don't have one...

This is a clean install? Why are you trying to make your own boot partition? If you do that it does not set the boot flag on that partition it's missing a few steps, please answer my questions and I may answer yours.

Your video card istill not getting enough power. It can handle easy stuff ok. But when you go hard it draws alot of power... suggest trying a psu swap with something that has more amps on the 12v rail than what yours has.

could be your power supply, swap it out and check again. updated drivers? run hwmonitor from cpuid and see your temps and usage for processor and video card. does your screens just go black with sound sometimes, or is it a full restart?

when you play games does the computer go black screens / no signal but you still hear sound and such?

is your physx option set to use the gpu or your cpu?
