When a player plays Cabal Therapy, when do they have to state which card
they choose for the spell-- upon casting/ announcement, or upon resolution?

Er, this doesn't say anything about "choosing a card". It does ask you to
-name- a card, so you choose a card _name_, yes. That occurs on resolution;
it's not one of the things the rules say to do on announcement, so you don't
do it then.

Some people will make the mistake of letting opponent know what they WILL
choose, before resolution, but nothing says you have to. Opponent will not
know, at the time they can respond, what card you will choose.

(Note that trying to "powergame" this by saying one card name before the
spell resolves, then picking another one on resolution, is Severely Frowned
Upon by judges, due to it being unsportsmanlike behavior.)
