Club3D AMD radeon 9650HD (coolsteam) dying

Dear All,

I have seen a weird thing with my PC during last month.

The monitor picture shifts right as can be seen on the pictures. It happens totally randomly, under load and without any load, sometimes days without any problems, then 5 times in a row. The PC usually remains usable, it means that the music will continue playing, mouse can be moved (weird since getting to the left side means going off from the right) etc. It clears up and everything is ok after restart or a sleep. Two times it has ended up with lines on the screen and total freeze (only remowing power from PSU helped, not even the reset or power buttons).

a) I have tried to change monitors, reset to default, power off/on monitors - the thing stays;
b) changed DVI cables, no difference;
d) changed ports on graphics card;
d) uninstalled drivers, installed different drivers (including ones from original disc)

I have run furmark and OCCT - works like a charm, wo any problems.

What is this thing?

CPU: I5 2500K (OC to max turbo 4.7 with a arctic freezer 13 PRO)
PSU: Chiftech 750W
Motherboard: Gigabyte GA-P67A-D3-B3 s.1155
