Depends on what quality you consider minimum,twitch recommends 6000kbps for 1080/60.Lag will depend on how good your provider is, even if you have multiple times 6000kbps upload if your ips sucks or your region has bad connections or too many people at once on the internet you could have lag.

Yes if you do get that consistently, when connected to twich, then you are ok.
If 12 mbps is only to your provider then maybe.
