Leap Attack fails???

I've had this happen a few times. I'll go through a good part of the
game using Leap Attack, mainly for travel but sometimes to go after
those guys with the underground attack in Act V, then suddenly it won't
work any more. Fortunately, so far, this hasn't happened when I've
needed a quick escape but...

This isn't just me is it?

Used to happen to me every now and then, re-entering the game would fix it.

It happens to me as well all the time, but not with Leap Attack but with
Charge. I usually re-enter the game and it works just fine.

I've seen it happen with Leap (not Leap Attack), including times when
I've needed a quick escape.
