Monitors sometimes just turn off, have to hard restart.

I have a Gigabyte 7850 2gb card
Two 1920 x 1080 monitors
and a Cooler Master elite 500w power supply.

It only happens when I'm playing games, like I had farcry 3 and bioshock on ultra and the screen did a funny multi coloured stripe monitor crash.

Now sometimes they just turn off during high graphic games, say there's no signal and I have to restart my computer by holding down the power button.

I have the fans set to 70% and up when my temp is at 40c or more, 80 at 50 and 90 at 60+ it doesn't happen often but I'd rather it not happen at all, I did a FurMark test and it didn't crash or anything so I'm not sure if it could still be related to overheating or just a faulty card.

If it's overheating, what could I do to make the cooling better? More fans? Thermal paste? Attempt to figure out watercooling? I'd appreciate you telling me like I didn't know anything about cooling a computer because, well, I don't.

I'm not sure how to test it though because it only happens under heavy load sometimes, like I said I played bioshock on ultra for a week before it did it once and then I finished the game without it ever happening again so I'm really not sure how I'd be able to test it properly.
 
I have a program that came with the graphics card installed and up on my second monitor at all times, it never seems to go above 60c when it crashes, it also monitors the GPU voltage and memory Voltage but if neither of them are raised when the crash occurs, what else could it be?
