Question Strange graphics port issue

I have been facing a strange issue with my GPU as well as on-board graphics card. I have an Nvidia k620 that was connected to a monitor through the DP port. But it suddenly stopped working for no reason. In order to troubleshoot the GPU and monitor, I connected the monitor to the on-board graphics DP port. It was working fine for a few days until one day the computer made that "device disconnected" sound prompt and the on-board graphics DP ports also stopped working.
Could anyone point me in the right direction what could be the cause of this? Could any weird PC power issues by frying the DP ports

System Specs:
Windows 10
Refurbished HP Prodesk 600 G1
Core i5 4570
16 GB RAM DDR3
GPU Nvidia K620
Stock HP 320W PSU
128 gb SSD Western Digital Green drive
500 gb HDD Seagate
Using a TV as a monitor, connected to the GPU's DP port through a DP to HDMI converter (which is fine, cuz it worked fine with the internal GPU DP ports.

Update your post to include full system hardware specs and OS information.

Include PSU: make, model, wattage, age, condition (original to build, new, refurbished, used).

Disk drive(s): make, model, capacity, how full?

Look in Reliability History/Monitor. Any error codes, warnings, or even informational events being captured that correspond with the times of the monitor problems.

Or any other problems for that matter?

Could be something wrong with video cable or monitor itself.
Try a different video cable,
try different video output - DVI or HDMI (whatever is available).
Try a different monitor.
I first suspected the GPU. I readjusted it in the Mobo slot but that didn't work.
Then I checked the mobo slot with another spare GPU. That worked fine. So the Mobo slot is ok.
I checked the k620 GPU in another older spare PC, the DP port didn't work but it's DVI port was working.
I could use the DVI port but I'm worried of it getting faulty just like the DP ports, in case they're getting fried for some other reason.

Where can I find the Reliability History/Monitor?

Windows tool.

Type "Reliability History" into the Search box (oval) usually located in the lower left screen corner.

That will lead to Reliability Monitor. Naming inconsistent.

There are other tools available within Windows as well: e.g., Event Viewer and Process Explorer.

Reliability History/Monitor being the most straightforward and end-user friendly.

Good starting point.
