Same fps with low and ultra graphics settings generally means that you are CPU bottlenecked. The i7 8750h is a pretty old processor and thermal throttling could also be in play here since it's a laptop. You should monitor your CPU temperature and check with HWInfo if you get thermal throttling. Also monitor your GPU load (if it's constantly lower than 95% it's not fully utilized so something else is limiting your performance).

Also, are you playing on the laptop screen or do you have an external monitor? What is the refresh rate of the display? If it's 60 Hz it would explain a lot. Are you sure you don't have any fps limiter enabled?

hi, 144hz laptop, vsync off, cpu temps same after cleaning and new thermal paste - 94-99c cpu temps, before same temps like this, i add new thermal paste grizly... clean dust, and still fot high cpu temp

Looks like the CPU is reaching it's max temp and slows down itself to prevent damage (thermal throttling). This is a common issue since laptops don't have the same cooling capacity as desktops. Did you make sure the fans are spinning normally (no grinding noise)? Do you keep the laptop elevated and on a flat surface?

You could also try a laptop cooling pad. I used one several years ago for an overheating laptop and it worked very well.
