When recording it is between 70-90% on GTA V

I have a Ryzen 5 1600 and a RX 580 8gb and I can't really get the sweet spot for the settings I'm using 720p scaled resolution with a bitrate of 4000 using the faster cpu preset with high profile however the footage looks really grainy/ pixelated.

As @Vic 40 said my assumption would also be the fan bearing due to you saying that it gets louder when gaming meaning the fan needs to spin more increasing the noise.

When did the issue occur, i hear that quite a lot of people are having a lot of issues with the new Windows update version 1909 and have a lot of varying problems due to this new version.

I would also recommend trying to set a custom fan curve after, the drop of frames could be due to overheating, so a custom fan curve can cool it down quicker than the auto setting.
