My computer is extremely loud grinding noise at start up. Then it just stops when I'm on my desktop screen. At first I thought it was the power supply then I switched it and same things. Can it be a hard drive failure? Or perhaps a fan failing?
 
Have you opened the case to try to isolate the noise? Could be the fan, but the time for guessing is over. To isolate the noise, disconnect the fans you can including the cpu heatsink fan (briefly) and listen carefully. Could just be a wire touching a fan. Reposition your wiring if you haven't checked it lately.

It sounds like the hard drive itself turning on is making the noise. If that is the case the drive should be replaced ASAP. In my opinion the moment a hard drive starts making any sort of grinding noise you are living on borrowed time with it
