Question Monitor screens go black when gaming, but I can still hear Discord and background sounds ?

When I play certain games like Dark and Darker, Tarkov, or even DOTA 2. At some point my monitors will go black, but I can still hear sound from my discord. I have to do a hard reset, this has been happening to me every since I sent my old card off to get it fixed and Nvidia gave me a refurbished one.

MBD: ASUS TUF GAMING Z690-PLUS WIFI D4
GPU: Asus RTX 3060 Ti
CPU: Intel Core i7-12700K
PSU: Rosewill CAPSTONE Series 750W 80 Plus Gold

Things I've tried,

1. reformatting computer, reinstalled nvidia drivers
2. reseated the graphics card

I have a hwinfo64 log, in which the final minute records the black screen crash as well. I don't see the gpu temps hitting max throttle levels, maybe someone else can tell me what is wrong?
