Solution
NEVER use anything but ASUS EZ Flash 2 to update your BIOS!!!

Use only the Flash Drive method and prior to updating the BIOS boot into the BIOS and press (F5) Load Optimized Defaults. The ROM file must be unzipped first.

BIOS P8Z68-V PRO/GEN3 (3202) - http://www.asus.com/Motherboards/Intel_Socket_1155/P8Z68V_PROGEN3/#download then unzip file, inside is P8Z68-V-PRO-GEN3-ASUS-3202.ROM

EZ Flash 2 is a built-in App to allow the BIOS to be upgraded and USB Flash Drive is the storage for the BIOS ROM file to be stored/accessed from with BIOS EZ Flash 2. The USB Flash drive must be formatted in FAT-32.
