Question when using my computer, my sound cuts out along with my screen cutting black and displaying hdmi no signal

Hi, when playing games and sometimes when returning to my computer after a while, I notice the screen being black and have no other option but manually restart my computer. When this occurs first the sound cuts out making a buzz static like noise through the speakers for about 3-4 seconds simultaneously with the screen going black displaying hdmi no signal. I tried a combination of hdmi cables and monitors so im sure they are not the issue. Cooling is also excelent and there is no consistency for the crash as sometimes it occurs when playing high requirement games such as the new cod and sometimes it occurs when playing a decade old game like medieval 2 total war. Driver updates are also likely not an issue, since ive manually reinstalled them and have used iobit driver booster aswell.

OS: Windows 10 x64
CPU: AMD FX- 6300
GPU: AMD RADEON R9 390
PSU: CORSAIR CX600M
MBOARD: ASUS M5A97 EVO R2.0
RAM: 2X4GB

Please someone help me identify the issue it has been occuring with this PC for years now, even after several reinstalls. As a final thing to add when playing or watching videos sometimes the sound just cuts out for like 3-4 seconds then comes back and everything back to normal. when I speak through my mic whilst this occurs, it still pick up my voice. This might be unrelated to my main issue but I thought i'll also ask.
