Question Computer freezing with new GPU

My GTX 760 broke a while back so i was using the onboard hd 4600 for a few months and there was no problem, now i installed my friend's radeon R7 250x and it runs pretty good, but every once in a while mostly in games it freezes completely, all processes go unresponsive and then after a few seconds the mouse stops moving too. the only way to fix it is to press the reset button. what is causing this? could i have a faulty PSU? is that why my last gpu broke? when it broke i was playing and everything just went black suddenly. appreciate the help

CPU: core i5-4690
Ram: 8GB
Psu: enermax triathlor eco 650w
GPU: AMD r7 250x
motherboard: asus z87-k

I would run HWInfo and look at your CPU and GPU temps as well as your +12,+5 and + 3.3 voltages.

I like my CPU temp under 70 and GPU temp under 80.

The voltages should be within 5% of nominal.
