[SOLVED] Grahpics card problem/crash while gaming

Hello everyone, I've been dealing with two very annoying problems for the last two months.. when playing games sometimes the screen either looses signal and the gpu fans speed up to 100% or the pc crashes and the screen shows one random color with lines.. This (color with lines) also happened once while viewing a simple website on chrome. I have tried reinstalling the drivers, reseating the graphics card, checked the pci-e cables but nothing fixed the problem. I've been using this graphics card for 4+ years and never had any simular issues before... it all started two months ago. Does this mean the graphics card is dead? What do you guys think?

MOBO: Gigabyte 970A-UD3P
CPU: AMD FX-8350
RAM: 2x Crucial Ballistix sport 4GB 1600MHz
GPU: Sapphire Radeon R7 370 Nitro 4GB
HDD: Western Digital Green 2TB
SSD: Corsair Force LE 120GB
PSU: TurboX 830W 80+ Bronze
OS: Windows 10 Home 64bit
 
Solution
My guess would be failing gpu but the only way to know is trying a new gpu. However you have a terrible quality psu that I would not risk a new gpu with, this could have caused the damage or reduced the life of the original gpu.
