Sometimes my PC gets a "unexpected I/O error has occured" When booting windows, But when I replugged the sata, It boots to windows normally.
And sometimes it freezes. Is it a hard drive failure ? or the ram ?

Try change SATA port, or SATA cable. Seems like a bad HDD connection to me.
