CPU: i5-2400
GPU: Gigabyte GTX 660 2X OC Windforce Edition
RAM: 16GB DDR3 Dual Channel
PSU: NZXT 750W 80+ Gold

I want to upgrade my GTX 660 to something like a GTX 960 with 4gb on it, but I'm worried the BIOS of my OEM Lenovo M91p won't support modern cards like that. The motherboard has a PCI-E 2.0 16x slot which isn't a problem but the BIOS is version 9HKT47AUS from 1/10/2012. As far as I know that's the latest BIOS for my board (Model 7034DD8), and I'm concerned that I can't go past a GTX 600 series GPU. The first time I upgraded was going from a Sapphire HD 5770 to an R7 270x, but the R7 didn't even boot so I went to the 660 instead and that worked. Later I was told by someone on these forums that my motherboard didn't support something like the R7 and likely wouldn't support anything above the GTX 600 series. But I've heard that AMD GPUs just have issues on OEM Lenovo M91p desktops. So do you think I can actually upgrade to something like the GTX 960 without issues?

You'd probably have to check with Lenovo forums to see if any users of your specific model of PC have had success.

Some newer AMD cards, for example, had a Legacy BIOS/UEFI switch on them so they could work with either older or newer systems. Some don't. I bought a Gigabyte R9 285, and it did not have such a switch, so, it wouldn't work on my Dell XPS 8300 (which had an i5-2320, so same generation of CPUs as your machine).

I have far less experience with Nvidia, but you might run into the same issue. My son now has that old XPS 8300 and it runs a 660Ti (an EVGA model) just fine.

I *think* 700 series cards should work, but I'm not entirely sure. You're right to be cautious, though. I would strongly suggest forums where there are Lenovo users, to see what their experiences were with 9xx and 10xx series Nvidia cards.

I checked and it seems like a GTX 960 works on a Lenovo ThinkCentre M81 so it might work on mine. I think I'll wait some for other people to say something here first though

I think I found my answer; it looks like the M91p only supports Nvidia series GPUs from the GT 420 to the GTX 980. So I might end up buying the GTX 960 4GB SC I've been looking at and see if it works. For anyone else with a problem like mine, here is the Lenovo ThinkCentre M91p Approved GPU list:
