The title is pretty self explanatory, every once in a while, whether I'm playing video games, watching a movie, etc (it seems to happen most often when playing games however), my screen goes black for a second or two before coming back on as if nothing ever happened. It's a relatively minor inconvenience, but it does get annoying if I'm, for example, playing a fast paced game and it causes me to die in it.

What would cause this? Is it my monitor? My graphics card? My DP cable?

Solution
Start by checking all external cables and connections. Power cables also. Ensure that all cables are fully and firmly connected.

Open the case and reseat the GPU. Likewise check that all internal connections, cables, and RAM are fully and firmly connected.

Try another known working DP cable.

Try another known working monitor or TV even. Determine if the disconnects stay with the computer or follow the monitor.

Look in Reliability History for error codes and warning that correspond with the disconnects.

You can also use Task Manager or Resource Monitor (one at a time) to observe system performance. You may spot some change that corresponds with the disconnects.
